Job Overview:
Join UnitedHealthcare as a Field-Based Care Coordinator – Long-Term Services and Supports (LTSS) and play a vital role in simplifying the healthcare experience, creating healthier communities, and removing barriers to quality care. As a Care Coordinator, you will be responsible for establishing person-centered, goal-oriented care plans that ensure individuals receive supportive, effective, and efficient services. This field-based position offers the flexibility to work remotely from your home office, with expected travel 2-3 days per week within a 30-60 mile radius.
Key Responsibilities:
- Assess, plan, and implement individualized care strategies that promote the most appropriate, least restrictive level of care
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
- Manage person-centered service/support plans throughout the continuum of care
- Communicate with stakeholders to ensure quality, coordinated care and services are provided expeditiously
- Advocate for individuals and families as needed to ensure their needs and choices are fully represented and supported
- Conduct home visits and in-person visits in various settings, including nursing homes, assisted living, hospitals, or homes
Requirements:
- Bachelor’s Degree (or higher) in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
- 2+ years of experience working within the community health setting in a healthcare role
- 1+ years of experience working with individuals with long-term care needs
- 1+ years of experience working with individuals receiving services on one of the home and community-based waivers in KS
- 1+ years of experience working with MS Word, Excel, and Outlook
- Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings
Preferred Qualifications:
- Licensed Social Worker or clinical degree
- Background in managing populations with complex medical or behavioral needs
- Experience with electronic charting
- Experience with arranging community resources
