About the position The Pharmacy Department at Sanford Health is seeking a full-time pharmacist to provide clinical pharmacy services focused on oncology medication management. This role involves working in an interdisciplinary team environment, either on-site, remote, or hybrid, to ensure the appropriateness of high-risk medication use for both inpatient and outpatient oncology patients. The pharmacist will also have opportunities to serve as a preceptor for pharmacy residents and participate in various committee work and projects to enhance oncology pharmacy practices. Responsibilities Provide clinical pharmacy services for inpatient and outpatient oncology medication management. , Review and verify all high-risk medication orders for appropriateness from clinical, financial, and policy compliance perspectives. , Participate in decentralized inpatient oncology team rounding and support clinical activities such as pharmacokinetics and TPN monitoring. , Conduct anticoagulation reviews, renal dosing, IV to PO adjustments, medication profile reviews, and antimicrobial stewardship for oncology patients. , Serve as a preceptor for PGY-2 Oncology Pharmacy Residency program and other pharmacy residents and students. , Engage in committee work and projects to elevate oncology pharmacy practice. Requirements Graduate of an accredited School of Pharmacy. , Thorough knowledge of drugs and their actions, as well as state and local regulations. , Hospital experience or residency preferred. , License in good standing by the State Board of Pharmacy in state(s) of practice or possess multi-state licensure privileges.
