Job Overview:
We are seeking a compassionate and detail-oriented 245D Designated Coordinator to join our team at Homebridge Care LLC, a leading provider of 245D services in Minnesota. As a 245D Designated Coordinator, you will play a vital role in overseeing and supporting our Direct Support Professionals (DSPs) in delivering high-quality services to individuals with disabilities. This is a part-time, remote opportunity with the potential to become full-time as the role expands.
Key Responsibilities:
- Develop and update individual program plans, including CSSP Addendums (CSSPA) and Individual Abuse Prevention Plans (IAPP), to reflect clients’ needs and goals.
- Ensure regulatory compliance with MN Statute 245D licensing requirements and legal standards.
- Monitor and review program records to ensure accuracy, completeness, and timeliness.
- Collaborate with DSPs, case managers, and families to deliver person-centered care.
- Lead client intake sessions, annual reviews, and compliance meetings to align care goals.
- Uphold HIPAA privacy and security guidelines to protect sensitive information.

Requirements:
- Bachelor’s degree in a related field + 1 year of direct care experience, OR
- Associate degree in a related field + 2 years of direct care experience, OR
- High School Diploma + 4 years of direct care experience.
- Familiarity with 245D requirements and the ability to apply them effectively.
- Strong organizational, time management, and communication skills.
- Ability to work efficiently in a fast-paced environment.
- Ability to pass a background study with the MN Department of Human Services.
- Reliable internet connection and tech setup for remote work.
