As a Case Manager RN, you will be responsible for telephonically assessing, planning, implementing, and coordinating care management activities to facilitate the overall wellness of our members. You will work closely with our care management team to develop proactive strategies that address complex health and social indicators, reduce risk factors, and promote optimal health outcomes.
Key Responsibilities:
• Conduct comprehensive assessments of members’ medical needs and develop personalized care plans to facilitate overall wellness
• Collaborate with healthcare providers, family members, and other stakeholders to coordinate care and ensure seamless transitions
• Apply clinical judgment to incorporate strategies that reduce risk factors and address complex health and social indicators
• Utilize case management processes in compliance with regulatory and company policies and procedures
• Consult with supervisors and other healthcare professionals to overcome barriers and achieve optimal health outcomes
Requirements:
• Active and unrestricted RN licensure in Ohio
• 2+ years of clinical experience
• Associate’s Degree or Nursing Diploma required; Bachelor’s Degree preferred
• Strong computer skills, including Microsoft Office (Word, Excel, Outlook)
