Key Responsibilities

Coordinate and schedule in-home care services for patients based on their individual care plans.

Assign and confirm caregiver appointments in alignment with patient preferences, needs, and caregiver availability.

Communicate professionally with caregivers, clients, and family members via phone, email, or messaging platforms.

Maintain accurate records of all scheduled shifts, cancellations, and client communications.

Respond promptly to same-day schedule changes, emergency requests, and cancellations.

Monitor caregiver attendance and availability and update the scheduling database regularly.

Collaborate with case managers, HR, and care teams to ensure efficient coverage and compliance.

Provide ongoing support to caregivers regarding their schedules, shift updates, and any concerns.

Utilize scheduling software and internal tools to manage workflow efficiently.

Ensure all scheduling complies with healthcare regulations and GracePath policies.
• Required Skills and Qualifications

High school diploma or equivalent (Associate degree in healthcare administration or a related field preferred).
• *
Strong command of English with excellent verbal and written communication skills.
• *
Proficiency in scheduling tools, Google Workspace, Microsoft Office, or CRM systems.

Ability to multitask and manage a high volume of scheduling calls or messages daily.

Strong customer service skills and ability to handle sensitive situations professionally.
• *
Familiarity with healthcare scheduling or care coordination is highly desirable.

Reliable high-speed internet and a quiet home workspace.
• Experience

Minimum 1 year of experience in scheduling, customer service, or administrative roles, preferably in the healthcare or home care industry.

Experience with remote work tools (Zoom, Teams, VoIP platforms) is a plus.
