As a Case Manager RN, you will play a vital role in ensuring the continuity of care for our patients, leveraging your nursing expertise to drive quality outcomes and improve patient experiences. Key Responsibilities: Utilize your nursing knowledge and skills to facilitate continuum of care, ensuring appropriate resource utilization and patient quality outcomes. Prioritize patient care needs, addressing emerging issues and developing personalized care plans in collaboration with patients, families, and caregivers. Coordinate treatment plans with the care team, triaging interventions and using protocols and pathways to optimize clinical outcomes. Monitor and coach patients using motivational interviewing and behavioral change techniques to maximize self-management. Maintain effective communication with physicians, hospitalists, extended care facilities, patients, and families to ensure seamless care transitions. Requirements: Active RN license in current state of residence, with the ability to qualify for additional state licenses as requested. 2+ years of nursing experience in a hospital, acute care, or direct care setting. Bilingual proficiency in Spanish, Cantonese, or Mandarin. Renal, Chronic Kidney Disease, or Dialysis Care experience. Computer proficiency, including MS Office and electronic health records. Preferred Qualifications: BSN degree. Certified Case Manager (CCM) certification. Diabetic educator experience. ICU, Cardiology, or Critical Care experience. Telephonic case management experience.
