We are seeking a compassionate and skilled Nurse Practitioner to join our Optum Home-Based Medical Care team in Pitt & Lenoir Counties, NC. Key Responsibilities:
- Conduct comprehensive assessments and develop personalized care plans for patients
- Collaborate with primary care physicians, interdisciplinary team members, and other healthcare professionals to ensure seamless care coordination
- Provide ongoing care and support to patients, including performing routine medical procedures and prescribing medications as needed
- Participate in ongoing meetings and quality initiatives to ensure the highest standards of care and promote clinical excellence
- Stay current on relevant medical and nursing research, technology, and related issues through ongoing education and professional development

Requirements:
- Completion of Nurse Practitioner accredited program
- Current NP license and state RN license
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
- Access to reliable transportation and compliance with Optum Motor Vehicle Safety policy
Preferred Qualifications:
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of CPT, ICD-10, and HCPC codes
- Knowledge of clinical standards of care and NCQA requirements
- Awareness of CMS guidelines, Medicaid/Medicare contracts, and benefit systems
